Did you tried, setting Compatibility Mode for quake3.exe to XP?
Reading quake log, it looks like issue with video mode set at 640x480 at 16 bit color, could you change it in the cfg file, i am not familiar with settings
i had this issue aswell , i fixed it by installing opengl , and then manually applying the opengl32.dll somewhere of which i can't remember , was a long time ago , but it was really hard and took a lot of googling.

3. What is your monitor maximum refresh rate? 60hz 75hz 120hz?

5. quake3 root folder's files list (printscreen is enough)

May be its because of 64 bit OS ,but i am not sure, i don't have how to test until monday
