RTFM is good, but far to often boring!
Figure u have a webserver installed with MySQL and PHP up and running?
I've only tried this on my local webserver which has PHP Version 4.4.1 and MySQL 3.23.49. Kind of old versions but phpBB2 works without fuzz
To create a db, phpmyadmin is a simple tool but lack the option of setting a password for accessing the db.

Sure you can set set passwords, there's a powerful user administration interface built into it. Just click "Priviledges" on the start page. You assign a password to a user, not to a database, and then you assign which users are allowed to do what in which databases.
