also i have no idea if it fixes the frameloss problem, so servers cant run 24/7 without restart.

That is just some tricks above 1.32b public source (homemade thing): no delay for sending dl packets (i suppose it will lag active players in favor of donwloaders) and forcing cl_allowDownload = 1 via system info bug. I would suggest include that forcing into e+ server code along with introducing map certificates that will allow only trusted maps to be forcibly downloaded.
with dfengine u can download if ur quake is connected to inet, u dont need to be on the server and it directly downloads from q3a.th.cx
dfengine also supports some other stuff which is definitly not ment for gaming

Did you interested in client updates?
Imo better make public servers on ioquake (http://ioquake3.org/improvements/)
Autodownload works fine with http, ftp and direct download from server.
Compatibility with regular quake.
And as u can see popular quake mods have own loader.
